Welcome to ECHP!

Extended Care Health Professionals, PLLC is a company that was founded by two board certified nurse practitioners on January 9, 2006. Our focus is in Long Term Care and Subacute Rehab. We have two goals in forming our own company.  We are able to provide patient care in a variety of settings and provide other interested nurse practitioners with the tools to do the same.

We collaborate with multiple health care professionals in nursing homes providing continuity of care that is individualized. We work as a team collaborating with physicians, nursing staff, pharmacists, administrative staff, dieticians, therapists, and social workers to name a few. Working in this manner allows us to provide quality care.
